Disclaimer, I am the author of “Stickler”:http://copiousfreetime.org/projects/stickler/

I saw the ‘Setting up and using an internal gem server’ section and thought you might want to know about Stickler, which is also an internal gem server, and I think may be a good example of a command line application written in a manner that showcases many of the tenants you describe in your book.

Cool! How is stickler different than, say, geminabox?

From the basic, push/install perspective, nothing, as they both implement the gemcutter api.

Stickler also comes with command line server app, to run a server wherever you want and a config tool to help you set it up as a rack app apache or nginx. Also, Stickler will mirror gems from rubygems.org if you ask it too.
