Unfortunately, there is no “one size fits all” answer. For many projects, being willing to blow away the development db at any time and start over is sufficient guarantee that problems will never make it into production. For others, three levels (development, test, production) may not be sufficient.

The right answer is: it depends. What is important is that you are aware that this is an area that merits increased attention to detail.

I wish I could say more, but the answer truly is “it depends” and “be mindful of this issue”.
